Severe Mismanagement and Underutilization of Funds

However, only Rs 9.78 crore was actually spent, reflecting a lack of commitment towards expanding the initiative as planned.
As a result of this financial underperformance, the Directorate General of Health Services (DGHS) fell far short of its target. By March 2023, only 523 clinics had been established, a stark contrast to the original goal of setting up 1,000 clinics by 2017. This shortfall has significantly affected the accessibility and effectiveness of healthcare services promised under the program.
Poor Quality of Medical Consultations
The audit findings also raise concerns about the quality of medical consultations at Mohalla Clinics. A staggering 70% of patients received consultations lasting less than one minute. This alarming statistic calls into question the credibility of the healthcare services being provided and suggests a fundamental failure in patient care and medical oversight.
Lack of Essential Medical Equipment and Medicines
Another critical issue highlighted in the report is the absence of essential medical equipment. Many clinics were found to be lacking basic tools such as pulse oximeters, glucometers, X-ray viewers, thermometers, and blood pressure apparatus. Additionally, the availability of prescribed medicines was far from adequate. The Essential Drug List (EDL) includes 165 essential medicines, yet a survey of 74 clinics found that none of them maintained a 100% stock of these vital drugs.
The report also sheds light on logistical inefficiencies in the supply chain for medicines. A significant proportion of required drugs were either not procured or were undelivered by vendors, despite official orders. For example, in the third quarter of the 2022-23 financial year, the District Drug Store in North-East Delhi requested 151 medicines, but only 79% were delivered. The situation worsened in the fourth quarter, with the supply rate dropping to 74%. Furthermore, delayed deliveries, ranging from three to six months, created additional disruptions, further compromising patient care.
Insufficient Oversight and Prolonged Clinic Closures
The CAG audit also highlighted a glaring lack of oversight in the management of Mohalla Clinics. Despite directives from the State Nodal Officer in May 2022 to ensure the timely supply and availability of medicines, little progress was made. Inspections of the clinics remained grossly inadequate, with only 2% of clinics in the selected districts being inspected between March 2018 and March 2023.
Moreover, the report found that 18 audited clinics were shut down for prolonged periods due to issues such as de-empanelment, staff resignations, and doctors taking extended leave. Some clinics remained closed for as long as 23 months, leaving residents without access to crucial healthcare services.
Financial Irregularities and ‘Ghost Patients’ Scandal
The report also brings to light financial irregularities within the Mohalla Clinic system. Despite the ambitious goals of the project, only 28% of the allocated budget was utilized. This lack of expenditure suggests either a failure in planning or a deliberate lack of intent to execute the project effectively.
Furthermore, a previous audit had already raised concerns about fraudulent practices in the system. It was found that 65,000 medical tests were conducted on ‘ghost patients,’ indicating possible corruption within the administration of the clinics. This revelation has further damaged public trust in the program.
Public Disillusionment and Political Fallout
The CAG’s findings have exposed the vast disparity between the AAP government’s promises and the actual performance of the Mohalla Clinic initiative. The project, initially hailed as a model of accessible and world-class healthcare, has instead become a symbol of inefficiency and mismanagement.
